Reason: Bald Spot On Fur Right Front Leg Near ChestAppointment Notes: has come up since last week apptHistory: - Presented for O noticing a wound around left cranial shoulder since vaccination last week- Indoor and outdoor cat - Has kept indoors since vaccination- Has stray cats around the neighbourhood, has previously been into cat fights- Only cat at home- Doesn't let O touch the wound- No limping or obessively licking at the wound- No vomiting/diarrhoea, EDDU, other wise O has no other concern.Physical Exam: Subjective- QAR. NervousObjective- BWT: 6.5kg. BCS 5/9- Vitals - MM pink + moist. CRT <2 sec. HR 200. RR 40. Temp 39.2- Cardiovascular: NAD. No murmurs or arrhythmias detected.- Respiratory: Clear bronchovesicular lung sounds in all quadrants.- Gastrointestinal: Soft and comfortable- Musculoskeletal: Ambulatory x4. Full musculoskeletal examination not performed.- Integument: 3cm wound around the distal right jugular around prescapular, evidence of healing, no discharge/heat/swelling present.- Urogenital: NAD- Otic: NAD- Ocular: NAD.- Neurological: Normal mentation. Full neurological examination not performed.- Oral: Mild dental disease. C1G0Assessment: - DDx: Trauma vs cat fight wound vs othersPlan: - Gently clean the wound with dilute chlorhexidine solution- Cephalexin 100mg PO q12hrs for 5 days- Monitor wound for discharge, bleeding, swelling- Recheck sooner if worsenDiscussion: - O would like to have the F5 vaccine next time, as Stan is mainly an outdoor cat- Advised not to get the F5 vaccine soon, as F3 was only given last week    Vital Signs
