Reason: Limping On Back Right LegAppointment Notes: Overdue remindersHISTORY:  Presented for assessment of a right hindlimb lameness. The owner first noticed the limp this morning, noting reluctance to get out of bed and not weight-bearing on the right hindlimb. The lameness may have started yesterday, as Yogi was less willing to run and play fetch in the backyard. He was reportedly normal after doggy daycare on Friday and during a walk on Saturday. There is no known specific traumatic incident. There was a potential yelp heard last night during play with a kitten (Mookie), but the event was not witnessed. Appetite and thirst are normal. No vomiting or diarrhoea reported.Current meds: noneVaccination: Not mentionedParasite Prevention: UTD. Gave NGS on Saturday.Pre-existing conditions: noneAccess to toxins eg. Rodenticide: none EXAMINATION:A targeted examination of the hindlimbs was performed.Body Condition Score: 5/9Pain Scale (0-4): 1/4Hydration status (%): hydration clinically adequateRespiratory: respiratory effort normal, no nasal dischargeNeurological: neurologically no significant findingsMusculoskeletal: Ambulates on all four limbs with a subtle right hindlimb lameness. When standing, appears to favour the left hindlimb. Palpation of the left hindlimb revealed no abnormalities. On the right hindlimb, there is discomfort on deep palpation of the lateral quadriceps region. No crepitus or obvious swelling detected. Range of motion in the hips, stifles, and hocks appears normal bilaterally. He shows some reluctance to have his paws handled on both sides, slightly more pronounced on the right.Integument/Ears: Paw pads are healthy with no visible wounds, cuts, or foreign bodies. Coat is very thick. No other significant findings.Lymph Nodes: NSFEyes: NSFDemeanour/Behaviour: Bright, alert, and responsive. PROBLEM LIST:- Acute, mild, right hindlimb lameness.- Mild discomfort on palpation of the right lateral thigh. DIFFERENTIAL DIAGNOSIS/ DIAGNOSIS:Suspected mild soft tissue injury (sprain/strain). DIAGNOSTICS:None ASSESSMENT:Yogi Bear presented with an acute onset of a subtle right hindlimb lameness. Examination findings are consistent with a mild soft tissue injury, such as a sprain or strain. Discomfort was elicited on deep palpation of his right lateral quadriceps. There are no signs of a more severe injury like a fracture or ligament tear. The history of playing with a kitten last night may be related to the onset of the injury. TREATMENT:  Pain relief was offered but declined by the owner at this stage.Should the owner change their mind, we can dispense some Metacam - Delayed prescription: 10kg dose of metacam SID with food for 5-7 days. The importance of strict rest was discussed. PLAN:Strict rest for 10-14 days. This includes no running, jumping (on/off furniture), or boisterous play (especially with the kitten). Leash-only walks for toileting purposes are permitted.Re-evaluation if the lameness worsens or does not improve with rest.Home into owner's care. CLIENT COMMUNICATION:Discussed the likely diagnosis of a mild soft tissue injury and the management plan. The owner understands the importance of strict rest for two weeks, regardless of whether pain relief is administered. Advised that giving pain relief can make the dog feel better and more likely to over-exert themselves, hence the need for even stricter confinement if medication is used. The owner has opted to manage with rest alone for now but was advised they can call to collect pain relief without another consultation if needed.
