Presented for: seizure breakthrough
History: 
- P has been on phenomav for seizure control and gaba PRN.
- P had one breakthrough last month and had 2 seizures last night, 20 mins apart, 90s each.
- quick recover from seizure. has been settled in well at home. EDDU normally, good appetite, V-D-. 
- P is back to normal at presentation
Medication given: phenomav 100mg; 3/4 tab morning and half tab night. O gave gaba BID after seizure

Physical Examination
BCS 6/9 & Demeanour BAR
Temp: 38.3
Cardio/Respiratory: HR  110 bpm, RR  panting. MM pink,  CRT <2 seconds
Thoracic auscultation: Clear bronchiovesicular sound, no heart murmur
Eyes: Clear, 3rd eyelid NAD
Ears: NAD, clean
Integument system: soft coat, free of ectoparasite, NAD
Lymph node: submandibular WNL, pre-scapular WNL, popliteal WNL
Dental score: 1/4, mild gingivitis
Abdominal palpation: soft, no pain elicits on palpation
Musculoskeletal examination: ambulation normal
Neurological examination: full exam not perform, mentation normal, proprioception normal

Medication request, pet not seen
Confirmed with owner afer phenobarb and gabapentin (medication request was for vetmedin which we are not on, suspect typo)
Bloods were done a month ago, spoke with Adrian and was due for health check in November so will count that as the check

presented for phenomav + liver bloods

phenobarbital: 50mg q12hrs (current dose 2mg/kg)
last seizure was a small one - about 2 weeks ago 
prior to that was 2 months ago 
O has no concerns about frequency of seizures - usualy every 3-4 months - adv can add other meds in but o not concerned currently so declined 
bloods taken 1 hour before pheno due 

To call O with bloods when through 

chem 17: ALP mild high 286 (ref 23-212)
Phenobarb: 108 within the ideal range
