Presenting for Blood pressure, Repeat bloods and UPC

- Current medications: Fortekor 2.5mg BID, Odansetron 2mg SID 
- Vaccinations and flea/worming tx: Utd

CV: NAD. No m/a auscultated
Respiratory: Normal resp effort & lung sounds bilaterally
Abdomen: Soft & pliable, no pain response observed upon palpation
Eyes/nose: Clean, no discharge, redness, or other abnormalities noted
Ears: Clean, no discharge, redness, or other abnormalities noted
Teeth: Grade 1/4 Mild plaque on molars 
Integument: Clean, healthy coat
MSK: Ambulatory. Normal gait.
Urogenital: No redness, swelling or discharge noted around penis/vaginal or anal area. Bladder small today, could not collect urine sample for UPC
Neuro: Normal mentation. Full neuro exam not performed

Borderline low HCT
Mildly elevated MCHC/RDW
Mildly elevated amylase
Moderately elevated BUN
Mildly elevated Creatinine
BP measurements (see other note) - Borderline hypertensive, consistent with previous measurements - stable 

Assessment:
Renal azotaemia
CKD managed well with current medication + Darbepoeitin injections q1month

Treatment:
Darbepoeitin 0.4ml SQ

Plan: 
Send O home with Urine collection kit - O to bring back in. Submitting urine for Gribbles UPC - Billed to file 
Emailed O BW results 
Scripts for Fortekor and Odansetron
Return in 1 month for next injection, booked in appointment
