VBSeen by Dr Emily Fell NSW V11811Consultation with Tony History:- Seen by EMF 17/02/26 for malassezia dermatitis + UTI. Tx = cytopoint 20mg, fluconazole 100mg PO q24 x 28 days, malaseb washes twice weekly, topical dermotic on lip folds + ear pockets, amoxyclav 22mg/kg PO q12 x 7 days. - Gave the antibiotics, bathing with human chlorhexidine. Also been applying neocort on her paws and around her vulva. Not been doing dermotic. Instructions were mis-interpreted from my previous notes. I wrote fluconazole 100mg PO q24 x 1 week past resolution of clinical signs, meaning to continue the fluconazole until 1 week past the resolution of clinical signs. The label says to START 1 week after clinical signs resolve. Have re-printed label. Examination:Temperament: ***Temperature: ***Cardiovascular: HR: ***, no murmurs or arrhythmias, femoral pulses strong & synchronous, oral mucous membranes pink & moist, CRT 1 second.Respiratory: Eupnoeic, no crackles/wheezes on auscultation. Eyes/ears/nose: CleanOral exam: Dental grade ***Peripheral lymph nodes: Submandibular & popliteal lymph nodes small & soft. Integument: Full coat, normal skin tent. Abdominal palpation: Comfortable & pliable.Gait: Readily ambulatory, no ataxia or lameness.Musculoskeletal: Able to right, rise and ambulate without difficulty. BCS 5/9.Neurological: Mentation & responsiveness appropriate, full exam not done.Rectal exam: Not performed.Urogenital: Clean & dry. Current problems:Assessment:Plan:Client communication: Vital Signs
