Reason: RadsHistory:C+ more since last visit. This morning since around 4am - been doing more single cough which before usually was a sequence of C+. Normal in between the coughs. Fasted this morning. Toileting been normal.Always have good appetite.Indoor only. Not utd w/ prevention or vax as indoor only. Examine:BARMM pink/moist CRT 1-2sMod to marked diffuse gingivitis. Mild tartar. HR=PR=190. No murmur/arrythmia noted.All lung fields sound clear. RR30. Normal RE. Abdo soft and comfortable.Plan:Admit for sedate, chest xrays and PAS.Sedation: Butorphanol 0.2mg/kg (0.15ml) + Medetomidine 0.01mg/kg (0.06ml) IM Maintainance: Alfaxalone slow IV titrate to effect Laboratory:Mod hyperglycemia BG 16USG 1.025Glu +Radiographs: 3 views thoracic- Trachea normal position, symmetrical width, no narrowing. - Normal cardiac sillhoutte. - Pulmonary parenchyma unremarkable, no increased soft tissue opacity. - No pleural effusion. ET tube 4mm used for intubation and gentle manuvering to trigger a cough - no cough triggered. Mild clear mucus at the end of tube -> cytology primarily lymphocytes, extracellular cocci/rods. Recovery smooth. Gurgling noted from URT, likely from the intubation. Assessment:- No obvious sign of chronic bronchitis/feline asthma and has ruled out other structural lesion. DDx: other infectious /inflammatory respiratory disease- Gurgliness post-op likely secondary from intubation. Plan: Check if O happy to send fructosamine - concern for early DM given risk factor, other ddx stress. Alternatively, check glucosuria in urine and repeat usg. Monitor for PU/PD, polyphagia. Reason: Asthma vs furballHistory: Since last hospitalised has been doing really well. Main concern is a cough or a furball. Has been more regular over the past three months. Can occur through the night. Not occuring every day, can be random. Eating well. No V+D+. Can be sensitive with his food. Energy levels normal. Goes back to normal soon after coughing. Indoors only. Worming meds - nothing. Examine: QAR, lovely boy. MM pink moist, moderate gingivitis minimal plaque. Eyes/ ears/ skin/ coat normal, LNs nromal, CV normal no murmur, chest clear in all fields, no increased URT noise, normal RR and RE in consult. Weight gain 1.5kg, BCS 8/9. Assessment: Video very much looks like a cough as opposed to a furball. Ddx asthma, lung worm, neoplasia, infectious (latter less likely based on signalment and indoors only)Recommend chest rads est $900 under sedation to investigate furtehr. Plan: Rads under sedation.
