Sedated 
Cleaned paw thoroughly 
Interdigital area between D4 and D5 of RF thickened mainly on dorsal aspect of paw, mild
No evidence of draining tract 
No evidence of sinus on palmer aspect of paw
Placed 0.2ml lignocaine in swelling 
Small incision made over most thickened region dorsal paw - flushed copiously - clear saline no puruelent material and no evidence of FB found 
- leave to heal by second intention 

Suspect interdigital cyst/furunculosis as currently no evidence of FB 
  
Plan:  Wound check in 10-14 days
RV sooner if any concerns 

Medication to go home with: extend carprofen course to a full 10 day course
Bathe paw SID with dilute chlorhex for next 5 days 

Discharging Vet: JW in person, SH over the phone

Dusty has been licking at his front paw for the last couple days. 
Today O noticed a large, reddened area between two of the toes that Dusty would not leave alone. 
Here today to have checked. 

EDVD: normal 
Current medications: nil 

Examine: 
Mentation: BAR 
Body condition score: 4/5
MM: P/M                
CRT: <2
HR: 92               
RR: panting 
LN: nad
Temperature: not taken 
Cardiovascular: no murmur, arrythmia 
Respiratory: no crackles, wheezes
Integumentary & lumps (measurement, location): 
- Swollen interdigital region between digit 4 and 5 of RFL, small discharging sinus on underside of paw
- Clipped, cleaned and gently expressed, only purulent material removed, suspicious of fb remaining. 

Problem List/Assessment: 
Swollen interdigital region between digit 4 and 5 of RFL, small discharging sinus on underside of paw.
DDX: fb v furunculosis v cyst v neoplasia v other 

Plan: 
Offered medical tx with possibility of requiring surgical explore if not improving v surgical explore first to r/o fb.
O has opted for latter. Advised e.collar and started on carprofen course. 
O has booked for surgical explore on Tues.
