03/03/2026 Diagnosis:Intermediate cell lymphoma, cytological diagnosis. CHOP 15 weeks, Three month post chemotherapy. History: Lucy has been quite lethargic over the past two weeks. She is restless at night time. Her appetite is reduced Thursday. Lucy had a one off vomit this morning. The family query whether she has a urinary tract infection, no change in her urination. The family have noticed Physical Examination findings: Bright, alert and responsive. Right mandibular lymph node is bilobular measuring 3 x 3.2 cm, left is similar in size. -left prescapular lymph node measures 2.8 x 2.8 cm. -Right popliteal lymph node measures 2 x 2 cm. Mm pink, crt < 2 secs, HR 108 RR 24 Temp 38.9 Wt: 25.55 kg. Laboratory results: Bloods performed two weeks ago and adequate for treatment. Treatment: 0.61mg vincristine was administered IV via a 22g catheter in the x vein using the closed injection Equashield system. Comments: We will see Lucy again in x weeks time.
