Reason: Seizure Episode On 2/3 Appointment Notes: would like to discuss medicationsPatient Information:- Poppy, Dog- Date of Examination: 03/03/2026Subjective:- Presented for seizure reassessment.- Experienced a seizure yesterday.- Another seizure occurred in December.- Had been well controlled on medication prior.- Seizure yesterday involved falling from the couch.- Shook and frothed at the mouth.- Vomiting was associated with the event.- Recovery was rapid.- Post-ictal behaviour included anxiety and fidgeting.- Showed interest in food post-seizure.- No other changes in general wellbeing.- No unusual behaviour prior to the seizure.- Currently on phenobarbital for seizure control.Objective:- General appearance is bright and alert.- Appeared slightly nervous during consultation.- Cardiovascular: Heart sounds normal. HR 123, no murmur- SKin/coat: WNL- A blood sample was collected.Laboratory:- Blood sent for phenobarbital level testing.Assessment:1. Seizure Disorder- Breakthrough seizures noted.- One seizure in December, one yesterday.- Currently managed with phenobarbital.- Assessment of control is required.Plan:1. Seizure Disorder- Check phenobarbital blood levels.- Decision on dose adjustment will be based on results.- If levels are sub-therapeutic, the dose will be increased.- If levels are within the therapeutic range, a discussion will be had.- This will weigh the risks of mild, infrequent seizures against potential side effects of increased medication dosage.- Owner to consider their comfort level with current seizure frequency versus medication increase.- Results expected in a few days.- Will contact owner with results to formulate a plan together.
