03/03/2026 Revisit reason: History: OR great up until 2 d ago when started to lick +++ again HAs been really bothered by bottom last 24 hrs Managing to get despite wearing EC diet black hawk & prime 100 - flavours vary was on salomon but recently switch to lab but had this flavour in the past Medications: gaba & traz for visit - has worked really well - much easier to examine today carp finished yesterday EDUF: all fine Mentation: bar, sedate, will still bark but short lived & not lunging & pretty amenable to exam today BCS (0-5): 3 HR: ne RR: 20 MM: p,m CRT: <1 Temperature: ne Dental Stage: ne Any additional findings: perianal erythema both glands still thickened blood tinged creamy discharge from LHS, nthign from LHS Assessment: ONgoing anal gland sacuclitis - allergies +/- autoimmune disease Treatment: clav 500mg bid On-going medications required: Y/N (use prescription tab for further supply): Plan: CP call in 3d time - if still irritated/licking lots then can dispense pred - script on file for O to collect form CL, RV here in 7 d time
