Patient exhibits chronic ear issues that are recurrent - episodes clear then return. Patient also requires nail clipping but is stressed during grooming procedures.</p>
<p>EXAM<br>Behavioural: Very stressed and anxious during examination. Required sedation tablets 2 hours prior to visit. Accepted chicken treats during examination which helped with distraction. Required nursing assistance for restraint during ear sampling.<br>Ears: Bilateral otitis externa with significant erythema and discharge. Ears described as very bad. Patient exhibited pain and resistance during examination.<br>Diagnostics: Ear cytology performed bilaterally - results showed Malassezia yeast infection with no bacteria present</p>
<p>ASSESSMENT<br>Bilateral otitis externa - Malassezia yeast infection confirmed on cytology. Chronic recurrent condition likely requiring ongoing management and prevention protocols.</p>
<p>PLAN<br>New Meds/Tx:<br>- Long-acting ear drops administered in clinic today. Medication remains active for approximately 30 days. Recheck appointment scheduled in 2 weeks to assess treatment response.<br>- Additional sedation tablets dispensed for future use during grooming and stressful procedures. Administer 2 hours prior to procedures.</p>
<p>Additional Discussion:<br>- Discussed long-acting ear medication as preferred option given difficulty with home administration<br>- Cost discussed - medication is expensive but more practical than daily home treatment<br>- Once current infection clears, discussed weekly preventative ear drops to reduce recurrence<br>- Advised sedation can be used for grooming appointments to reduce stress<br>- Recheck appointment in 2 weeks to monitor treatment progress</p></html>
