03/03/2026 HISTORY - Develops small, transient skin lumps described as tiny, raised, smooth, and pink, disappearing after 3-4 weeks - O requests dental check whilst here EXAM Skin: Found only 1 of the masses in skin at L flank. Was very tiny, 2mm diam, but looks like small sebaceous or follicular cyst. Other masses have resovled. Rest of skin looks very good Ideal body condition NAD on abdo palp NAD on thoracic ausc NAD on manip limbs Teeth: Localized DC on both UPM4s and UM1s. Tartar on LHS UM1 is over gum line adn localised mod gingivitis in this area. and gingivitis on the very back teeth, tartar overlapping gum line on the very back tooth Ears: Clean and healthy DIAGNOSIS - Follicular or sebaceous cysts for skin lumps - Localized dental disease with gingivitis and tartar buildup PLAN 1. Monitor skin lumps; no immediate intervention unless they grow larger or rupture 2. Dental cleaning under Grade 1/4 dental plan, including general anaesthesia, IV fluids, scaling, polishing, and probing of teeth 3. Apply dental gel daily to affected gum areas after cleaning 4. Recheck dental health in twelve months or sooner if problems persist 5. Schedule dental procedure for next Thursday, the twelfth
