Diagnosis Other relevant history/comorbidities:Chemo protocol:Treatment number: x/totalHistory since last visit:-     Appetite-     Thirst-     Urination-     Defecation-     Vomiting-     Other clinical signsHas had a few bouts of diarrhoea which O thinks due to dietary indiscretions (query eggshells), 3 weeks ago O was in Perth, had diarrhoea, started antibiotics which cleared up.  Hospitalised with tachypnoea and Dx heart failure - approx 2 weeks ago, injectable diuretics and home with frusemide.  8kg on discharge, had been 8.8 prior.Appetite now is back to normalBreathing is good.  SRR when he rests on the floor is OK per O had been close to 60 before hospitalised, he is around 40 but this is 'normal' for him.
