03/03/2026 Diagnosis: Lymphoma, intermediate to large cell lymphoma, B cell on ICC. Diagnosed in December 2024. Relapsed approximately 4-5 months post CHOP. Recommence CHOP. (#2) Week 19 mitoxantrone.   

History: Theo tolerated his last treatment well, there was no vomiting nor diarrhoea. There are no concerns with his well being. 

Physical Examination findings: Bright, alert and responsive. Oral examination notes some minor dental disease, otherwise unremarkable. Heart and lungs unremarkable. Peripheral lymph nodes palpating normally. Abdominal palpation soft and comfortable. 
Mm pink, crt < 2 secs, HR 130 RR 20 Temp 37.9 Wt: 14.05 kg. 

Laboratory results: CBC performed in house with manual blood film assessment.

Platelets:199

Manual differential count:  Neutrophils 72%, Lymphocytes 24%, Monocytes 4%, Eosinophils 0%.
Platelets: clumped and adequate
Morphology: Normal 

Treatment: 2.9mg mitoxantrone (5 mg/m2) was administered IV via a 22g catheter in the x vein using the closed injection Equashield system.

Comments: It was a pleasure to see Theo today for his last scheduled treatment. We will see Theo again in 1 months time including lymph node assessment and full bloods.  Please feel free to call or email if you have any further queries regarding Theo.
