HISTORY
Presenting complaint: Lump check
Diagnosis: SC mass RFL near olecranon

History:
- Generally well and no abnormalities detected at home except O has noticed a swelling/mass near R elbow. First noticed a few days ago. Seemed to pop up quickly.
- Does not seem painful when touched but Tyson has been limping on the RFL.
- Otherwise well at home. EDUD normally. No vomiting, diarrhoea, coughing or sneezing. O has no other concerns today.

Clinical Examination:
Demeanour: bright, alert and responsive. Limited examination today due to patient temperament.
MM: Pink and moist, CRT not examined
Eyes: NAD
Ears: NAD
Nose: NAD
Dental: NE
BCS: 5/9
Heart: NE
Lungs: NE
Pulses: NE
Abdomen: NE
Skin: Approx round, semi-firm, non-painful SC swelling/mass just proximal to RFL olecranon (~4-5 cm diameter). Thick calluses on both elbows.
Lnn: NAD
Musculoskeletal: Mild RFL lameness.
Urogenital: NE
Temp: NE

Diagnostics
- Unable to perform FNA today as highly reactive to needles. Requires sedation

Assessment/Differential Diagnoses
- Swelling/mass near RFL olecranon: ddx traumatic/pressure-related (hygroma/olecranon bursitis), inflammatory/infections (septic bursitis, abscess, FB granuloma), neoplasia (STS, MCT, other)

Treatment
- Paracetamol 500 mg PO BID 5 days

Plan
- Discussed differentials with O, including hygroma vs neoplasia.
- Recommended sedation for FNA +/- blood work for general health screen as temperament limits PE/diagnostics.
- Booked procedure for next Monday. O to call sooner if thinks additional pain relief is required.
