02/03/2026 Reason: Skin History: Went to groomers yesterday, groomer advised upon shaving they noticed many crusty lesions on belly, suspecting hotspots Not particularly itchy, licking occasionally but not excessively, maybe only here and there No history of skin allergy or hotspots prior EDDU well, no CVSD. UTD PPV with monthly chew, unsure of brand Vital Signs: Temperature: not taken Heart Rate: 140 bpm Pulse rate: 140 Pulse character: strong and synchronous. Respiration Rate: pant. MM: pink, moist CRT: <1 sec Dehydration assessment: Euhydrated MM moist no skin tent PHYSICAL EXAMINATION: Appearance: BAR; BCS 5/9 Eyes: nad. no redness, discharge, blepharospasm. responds to visual stimulus Ears: nad. no debris or inflammation Nose:nad Oral Cavity: mild tartar upper gumline at canines, rest nad Cardiovascular: Auscultation no murmur detected ,pulse quality normal ,rhythm regular Respiratory: RE normal , auscultation normal Abdomen: soft and comfortable Musculoskeletal: ambulatory all 4 limbs Neurological: appropriate mentation Integument: circular crusty lesions along ventral abdomen, lesions ranging from 0.5-1.5cm diameter, no erythema. woodslamp negative; sticky tape prep intracellular cocci noted Peripheral LNs:Normal size, symmetrical Urogenital: not assessed Rectal exam: not examined Assessment/Problem List: 1. Crusty circular skin lesion DDx: infectious (bacteria vs. fungal) vs. underlying CAD vs. hotspot vs. immune mediated vs. other Plan: Amoxyclav 250mg 1/2 tab PO BID x14d Pred 5mg 1/2 tab PO BID x3d, then PO SID x3d, then PO EOD until finished Gently clean lesion with dilute chlorhex BID x4-5d Recheck INI
