Presenting for: Follow-up for colitis with bloody faeces that had previously improved but has recurred over the last couple of days.

Historical Conditions:
- Previous episode of colitis with bloody faeces that improved with treatment

Diet:
- Currently eating chicken and rice
- Went off food yesterday but eating again as of 3 hours prior to consultation

V/D/C/S:
- Bloody faeces returned yesterday - described as mixture of fresh red blood and dark/black blood
- Diarrhoea present for last couple of days
- One small vomit yesterday
- Faeces described as liquid brown consistency when client went to shop yesterday
- Patient noted to drip faeces during the night

Current Meds:
- Previously on Pro-Kolin paste but supply finished

EXAM
Mentation: BAR
Wt: 4.5 kg (previously 4.6 kg - minimal weight loss)
T: 38°C (normal)
Abdominal: Palpable discomfort on abdominal palpation, gassy belly with gurgling and fluid noted
Rectal: Rectal examination performed - faeces soft and brown with small amount of mucous present, no blood or black stool observed at time of examination

ASSESSMENT
Recurrent colitis - Patient had previously improved but symptoms have returned with bloody diarrhoea and vomiting. Given the recurrent nature, infectious causes less likely. Possible underlying causes include dietary indiscretion or other gastrointestinal conditions requiring further investigation.

PLAN
Diagnostics:
- Blood test approved to assess WBC, RBC, kidney and liver function to evaluate for systemic causes of gastrointestinal signs
- Abdominal ultrasound to be scheduled for future appointment

New Meds/Tx:
- Pro-Kolin paste dispensed to continue supportive gastrointestinal treatment

Additional Discussion:
- Total cost for today's recheck, Pro-Kolin paste and blood test: $466.25
- Blood sample collection will require shaving small patch of hair on neck or leg
- Ultrasound appointment to be scheduled separately
