<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>History:<br>Approx 1 week ago, O noticed sore LH digit 2 - nail looked fractured. <br>He has been licking it since ++ <br>EDDU normal. <br>Occ toe touching lameness when standing & doesn't want to walk >100m. <br>Otherwise well. No current meds. <br><br>Examine:<br>BAR <br>HR/PR = 100, MM = p/m CRT <2s. Auscultation: Normal, no murmur or arrhythmia. <br>LH digit 2: Fractured nail, causing cuticle exposure. Mod amt purulent d/c at nail base & mod swelling/inflammation nail bed & interdigital skin. Can see proliferative tissue ventral to nail - will need to investigate this. <br><br>Plan:<br>Admit for sedation, proper exam & clip/clean/bandage LH.<br>Will need systemic Abs & NSAIDs. <br><br>Procedure Notes:<br>Sedated with 1.2ml meth/0.4ml medetomidine IM. Provided moderate sedation <br>Digit clipped of fur, significant purulent exudate, erythema and bruising both on affected digit and interdigital space<br>Cleaned with diluted chlorhex, followed by saline flush. <br>Coated in flamazine cream, then bandaged with melonin, conforming, vet wrap and elastoplast. <br>Reversed with 0.4ml atipamazole IM<br><br>Recovered uneventfully<br>TGH with meloxicam and amoxyclav for 10 days<br>Bandage removal and recheck in 3 days time, keep dry and clean. Remove earlier if wet/dirst<br><br></p></html>
