<p>PRESENTING COMPLAINT: <br>- Intermittent soft stools<br>- Eye discharge<br>- Ear discharge</p>
<p>HISTORY:<br>EDDU: Intermittent soft stools, no pattern identified, no blood or mucous, every few days. - Scavenges random things off ground, - No vomiting- No access to human food or rubbish<br>- Eye discharge noted around eyes<br>- Ear discharge present, previously much worse, owner has been cleaning regularly. not particularly itchy <br>- Black discharge around nose area, cleaned daily. no sneezing or coughing <br>- Energy levels normal, very active<br>- Indoor only cat. living with adult cat- getting along well <br>had 3x kitten vax already, first adult booster is due Nov 2026</p>
<p>Parasite prevention used: NexGard (green boxes)<br>Current diet: Raw canned food, kitten food, occasional homemade chicken breast with liver and pumpkin</p>
<p>HR: 160</p>
<p>EXAMINATION: <br>Eyes: Mild discharge, appears normal tear production, no inflammation or excessive blinking<br>Ears: Waxy discharge present, improved from previous condition<br>Dental/Mouth: normal occlusion, no dental dz, no cleft palate <br>Chest: Heart sounds grade 1 systolic HM heard to L comes and goes/ inconsistent. no arrhythmias heard, HR 160. femoral pulses SSS. lung sounds clear <br>Abdo: NAD, no abdominal pain on deep palpation, anus clean<br>Demeanour: BAR, affectionate</p>
<p>PATHOLOGY: Ear swab positive for Malassezia mild both ears</p>
<p>PROBLEM LIST:<br>- Intermittent soft stools<br>- Ear infection with Malassezia<br>- Possible heart murmur<br>- Eye discharge</p>
<p>DDx:<br>- Dietary sensitivity causing soft stools<br>- Malassezia ear infection secondary to immature immune system<br>- Innocent heart murmur vs underlying cardiac condition<br>- Normal tear production</p>
<p>ASSESSMENT: Young kitten with ear infection likely due to immature immune system. Soft stools possibly related to dietary variety or sensitive gut. Possible heart murmur requires monitoring to rule out underlying cardiac issues.</p>
<p>TREATMENT in consult room/Meds dispensed:<br>apex pmp - Ear drops 3 drops into both ears twice daily for 14 days</p>
<p>VET TECH actions:<br>Generate client summary in VetNotes <br>Email handout: none needed</p>
<p>PLAN: <br>- Consider probiotics to help gut health, consider strict bland diet. <br>- Complete ear drop treatment course- RV 2wk reswab<br>- Monitor heart murmur- RV to recheck, if persistent, discuss echo recommendation</p>
