<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>Patient Information:<br>- Vo<br>- Dog<br>- 9.5 years old<br>- Male<br><br>Subjective:<br>- Nighttime panting, restlessness, excessive drinking, unable to settle<br>- Husband takes Vo out during the night due to pawing and knocking the bed<br>- Owner has moved out of bedroom due to sleep disturbance<br>- Symptoms ongoing for approximately one year<br>- Trazodone previously trialled - caused frantic behaviour, digging under bed, broke items; ceased immediately<br>- Appetite reduced - sometimes eats independently, often requires hand feeding to start, now only one meal per day, sometimes not eating until 9pm<br>- Drinking pattern: minimal drinking until late afternoon, then drinks heavily<br>- Separation anxiety noted - when owners return from outings, Vo is worked up, salivating, panting, water spilled on floor<br>- Head shaking noted<br>- Gait abnormal - walks slowly or runs, no in-between; gait uncoordinated, trips on one leg, occasional knuckling<br>- Staring into space noted<br>- Gabapentin 300mg TID for back pain and muscle spasms - on this for approximately 18 months<br>- Gabapentin takes approximately 2 hours to work, effective for 3-4 hours, then panting and restlessness return as next dose approaches<br>- Slipped disc in neck approximately 4 years ago - treated with steroids and meloxicam at the time<br>- Gabapentin started approximately 2 years ago<br>- Symptoms were severe enough at one stage that euthanasia was considered<br><br>Objective:<br>- Blood sample collected for in-clinic analysis<br><br>Assessment & Plan:<br>1. Nocturnal Restlessness and Panting<br>- Differential diagnoses: cognitive decline, pain-related, gabapentin side effects, underlying systemic disease<br>- Blood test to assess for underlying conditions<br>- Results to be reviewed and discussed with owner<br><br>2. Abnormal Gait and Knuckling<br>- Possible association with gabapentin or spinal disease<br>- To be further assessed following blood results<br><br>3. Reduced Appetite<br>- Possible gabapentin side effect<br>- To be monitored<br><br>4. Increased Drinking<br>- Pattern of minimal drinking until late afternoon then heavy drinking<br>- Blood test to assess for underlying cause<br><br>Blood test shows normal parameters for haematology, biochemistry and t4 and cholesterol. <br><br>Plan: <br>Offered tx trial to owner of pain relief or tx for cognitive decline - o thinks cognititve decline. <br>Gave vivitonin to give a reduced dose of 1/4 tablet bid for a week and if needed can increase to 1/2 tab. <br>O will also start giving galliprant that they have at home. <br>Recheck in 2 wks.</p></html>

Follow up call post lumpectomy and change of antibiotics since 5th March. O informs me the wound is looking so much better and happy with how Viggo is progressing.
