<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>P presented hx of ocular irritation since this morning <br>O reports rubbing eye, off food, hiding, not wanting to go outside - very unlike them <br>O reports no vomiting/diarrhoea, no coughing/sneezing <br><br>CE: <br>P very quiet in consult; sulking around, mildly interested in treats <br>Quit head shy, blepharospasmic left eye, some mild discharge in medial canthus <br>MM pink and moist, CRT <2s - some dental disease present <br>SM lnds bilaterally symmetrical and not enlarged <br>Thoracic auscultation normal - No crackles/wheezes/murmurs or rales. Sinus arrhythmia, HR increases with breathing <br>FP strong and steady<br>Abdominal palpation soft and non painful<br>Rectal temperature normal (39.2)<br><br>STT >15 left eye <br>FLN tested but third eyelid coming across eye and unable to fully examine globe <br>Episcleral vessels engorged <br>Glimpsed part of pupil; potential hyphaema/uveitis - cloudiness seen in anterior chamber <br>Proxymetacaine drops administered on eye to take cytology swabs - results to follow <br><br>DDx: <br>Glaucoma <br>Uveitis <br>Conjunctivitis <br><br>Do not have tonopen here to measure IOP <br><br>Discussed risk of acute glaucoma with O - advised is an emergency - risk of eyeball rupture <br>O happy to be referred for assesment at north ryde vets - opthamology <br>Called SASH North Ryde - reception confirmed that IOP measurement will be done as part of emergency consult <br>Arranged referral for today - O happy with plan, aware that may be severe conjunctivitis but better safe than sorry re. glaucoma <br>Advised consultation fee of 350AUD - O happy with this. P also insured <br><br>Administered 0.2mg/kg meloxicam sq between shoulder blades as P very sore<br><br>Conjunctival cytology showed cocci - no IC bacteria, unsure re diagnostic value </p></html>
