Reason: Chocolate IngestionConsultation Time: 01/03/2026 20:34History:Presented for ingestion of chocolate cake approximately one hour prior to arrival. The owners reported that Tilly consumed a quarter of a homemade cake. It was clarified that the entire cake contained 30 grams of cocoa powder, meaning the ingested dose was approximately 7.5 grams. There was no other type of chocolate, such as baking or milk chocolate, in the cake, only icing. Tilly is otherwise a healthy dog and is not on any regular medications. Subjective:Bright, alert, and responsive on presentation. Panting but appeared comfortable.Objective: Temp: 39.2C  MM: Pink  CRT: <2 seconds  HR: 140 beats per minute  FP:  SSBCS: 6/9  Wt: 8.9 kgCV: No murmur or arrhythmia detected.RESP: Normal bronchovesicular sounds, no nasal discharge, normal respiratory effort.RR:  PantingGIT: Comfortable on abdominal palpation. U/G: No preputial discharge Neuro: Appropriate mentation.M/S: Ambulatory.Integ: Normal haircoat.Ophth: Pupils equal and appropriate. LNs: Palpably normal.Problem List:- Recent ingestion of chocolate cake.Assessment:The ingested dose of cocoa powder is sufficient to cause gastrointestinal upset, such as vomiting and diarrhoea, and an increased heart rate. Inducing emesis was appropriate to minimise the risk of toxicity. The risk of significant toxicity is now considered minimal since a large amount of the cake has been vomited. Activated charcoal has been administered to adsorb any remaining toxins. Monitor at home for GI signs, lethargy, not eating. Treatment:- Apomorphine: Half a tablet was administered onto the gingiva to induce emesis.- Emesis was successful, and a large quantity of chocolate cake was produced until only chocolate-coloured liquid was being brought up.- Metoclopramide: 4.45 mg (0.5 mg/kg) was administered via SC injection to cease further vomiting.- Activated charcoal: 45 ml mixed with food was administered orally to bind any remaining toxins.Plan:Discharge plan:- A bland diet is recommended for this evening and tomorrow. This can consist of boiled chicken breast and cooked white rice.- A small meal can be offered tonight if she is interested in eating.- The normal diet can be reintroduced tomorrow evening.- The owner was advised to monitor for any signs of inappetence or further vomiting.Client Communication:Communication was established with the owner in person and with his wife via telephone. It was explained that Tilly had successfully vomited a large amount of the ingested cake. The risk of toxicity was discussed and explained to be minimal given the dose and successful decontamination. The plan to administer a reversal agent for the vomiting and activated charcoal was discussed and consented to. The owners were advised that Tilly would likely have black or dark faeces for the next one to two days due to the charcoal. Discharge instructions regarding diet and monitoring were provided and understood. The owner had no further questions.    Vital Signs
