Presented for bravecto quantum + nail trim. EDDU. No Vx/Dx notedDiet: Urinary dietPara: UTD EndogardMeds: NoConcerns: NoPE: Cardio: Regular rate and rhythm, no murmur detected. Resp: No wheezes or crackles auscultated Oral: CRT <2s, pink and moist MM. Dentition g1Eyes: Bilaterally symmetrical, clear and no presence of discharge Ears: No erythema or discharge noted Integument: NADPlan: Vaccinated with bravecto quantum SQDue 20.2.27Discussed vaccinations, aware overdue KC, would prefer not to do as a low risk dog and worried regarding Rx vaccination reaction. Will return for titre testing later this year.

Diet: Urinary Prop: Milbermax (3 monthly) + Bravecto (3 monthly) Meds: None Exam:Eyes/ears: No discharge noted. Eyes clear.Oral: MM pink and moist. Minimal build up present. Cardioresp: No murmur or arrhythmia. Lung sounds clear.Integ: Coat in good condition.MSK: Ambulating well, no pain on joint palpation, good range of motion.Plan: Given SR12 0.27ml SC. Next due in 12 months. Discussed with O due titre testing +/- C5 vaccination next year. Explained O bloods take 40 minutes to run so will do when booked for SR12 and then bring back another day if needing vaccination as needs antihistamine 10 minutes prior.
