02/03/2026 Hx: here for review of AG abscess
O reports thinks infection cleared but still a lot of swelling around the anus. O reports poo is a bit flattened & skinny. 
O has tried psyllium husk, pumpkin & anal gland chews. Now using some sardines which has helped a little bit

PEx:
BAR
CVS: mm pink, moist. No murmur or arrhythmia
Resp: normal lung sounds bilaterally
Abdo: comfortable 
Rectal: L AG full of thick gritty material. R AG still very thickened, extends about 2cm internally and about 2cm in thickeness. AKT squeezed and blood material came out. Very painful


Assessment:
Anal gland abscess --> partially resolved. Possible concern for adenoma/adenocarcinoma given enlargement 

Plan:
Extend course of amoxy-clav & meloxicam for 10 days 
Review in 10 days with AKT

If concern about neoplasia consider abdo US first to look for enlarged LNs & consider desexing & removal of any enlarged inguinal LNs at the time of AG removal
