HISTORY presented for potentially swallowed cotton ball ~21:30. Acting strange since - coughing, eating grass, trying to vomit. - O got home around 9pm - possibly ingesting stuffing of soft toy then went out to eat grass - a few times tried to vomit - retching but then nothing came out - 9-915pm - offered some yoghurt after then the retching stopped - ate his normal dinner (around 730pm) - no issue - passing quite a lot of gas (abnormal for him) was in the backyard for 10 hours today no diarrhoea, no vomiting no access to known toxins 1 week ago backyard sprayed for bull ants --> has rained in the past few days no long term meds known to have occasionally seasonal allergies RV = Greensborough vet EXAM BAR, nice dog loves treats HR 108 no murmur or arrhythia with synchrous pulses RR panting lung sound clear bilaterally Temp 38.9 Abdomen soft and comfortable MM Pink and moist CRT <2secs rectal - normal stools ASSESSMENT not significant finding on PE PLAN monitor at home for further GI signs no medication indicated COMMUNICATION plan as above

SUBJECTIVE: presented for ear wound check post-op Appointment for IA/NBH, but IA in surgery, NBH not available - advised suture removal OBJECTIVE: Very BAR Ear wounds better than last week ASSESSMENT: Sutures x 3 in situ, but not apposing skin - TREATMENT PLAN: Sutures removed w/ advice from NBH COMMUNICATION: as above
