PRESENTING COMPLAINT: 

HISTORY: 
Once weekly - mild flare. Off food for 12-24h. Occ vomiting. Diarrhoea infrequent. Went to Newport for Prokolin. Diarrhoea daily for 4-5 days. 
Last week a few vomits (bile, saliva), few mucoid stools. 
Able to manage most bouts at home. 
Between bouts - bright, eating well, stools normal.
No obvious triggers. 
Urinating normally.
Good energy levels


Diet: Prime 100 kangaroo & pumpkin (wet & dry) - fed bid

Current medications: 
*Telmisartan 6 mg PO SID 1.5mls of 4mg/ml
*Ondansetron PRN
* Cyclosporin 0.1ml (10mg) PO sid (3.5mg/kg) > increase to 7mg/kg so 0.2ml PO sid (max dose 0.25ml) > if improvement is observed could trial tapering back to 0.15ml PO sid over time.
* Prednisolone 1mg PO sid (pulse with 2mg PO sid for 3 days if unwell)

No longer on fish oil... stopped one month ago when she was sick.


PHYSICAL EXAM:  HR 150bpm. Temp 39.4
Multiple sebaceous adenomas on trunk, largest one on medial right thigh. Irregular old scar on tongue tip (grooming injury). Bilateral senile nuclear sclerosis. BCS 5/9. BW 2.76kg (stable)

DIAGNOSTIC TESTS: 
BP 140 size 2 cuff RFL sitting
Temp 39.4
PCV/TP 48/84, serum cloudy
STandard profile
USG 1.036, pH 7, proteinuria (3+), dipstick otherwise unremarkable.
