Reason: Loose Stool Post Vax/worming On 22/7Appointment Notes: ggHistory: On and off loose stool since puppyHad vaccination on 22/07 and deworming and NGS. O noticed a speck of blood in faeces this morning along with usual loose stoolMiso has been on raw diet for his entire life, O started mixing small amounts of RC puppy kibbles. Also feeds bones daily. Examination: Demeanour: BARHeart: 100 T 38.2Resp: 30BCS: 5/9MM: p+m, CRT <2Mouth: C0G0, normal occlusion, no cleft palate MSK: able to ambulate well, normal gaitNeuro: normal mentation, no obvious cranial nerve deficits or ataxiaEyes: clear, normal menace response and palpebral reflex, responds to visual cuesEars: no erythema or lesions, not painful or warm on palpationAbdo: no pain on palpationInteg: clearLNs: normally palpableAssessment: Chronic loose stool ddx food intolerance vs gastroeneteritis vs parasites - Likely food related as currently on raw diet + bone + kibbles daily - Recc slow transition to kibbles/wet food from raw diet. Long discussion around nutrition- Owners are dedicated to find what food is causing the issue, recommended commencing a strict diet trial with hydrolysed kibble for 4 weeks to see if any improvement which will confirm food intolerance. Then, do further diet trial with different kibbles/food to find the right one.Treatment: Consent has been given by the client to proceed with this treatment - YesPlan:Start on prokolin 3ml BID O to monitor, revisit if concerned .......................................................................................................................................................................................................................................................................................................................Examination: Demeanour: BARHeart: 100Resp: 30BCS: 5/9MM: p+m, CRT <2Mouth: C0G0, normal occlusion, no cleft palate MSK: able to ambulate well, normal gaitNeuro: normal mentation, no obvious cranial nerve deficits or ataxiaEyes: clear, normal menace response and palpebral reflex, responds to visual cuesEars: no erythema or lesions, not painful or warm on palpationAbdo: no pain on palpationInteg: clearLNs: normally palpableAssessment: Treatment: Consent has been given by the client to proceed with this treatment - YesPlan:O to monitor, revisit if concerned--- 27/07/2025 13:18:27 PM WAL:Examinations - Resolved - Follow up text sent AW Vital Signs
