<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><div> <div> <p><strong>HISTORY: presetned with 3 days Hx of V and not eating<br>no access toxins but pest man came 1 day ago<br>will rip up house and so may have eaten foreign material<br>unknown drinking and urination and defaecation (does have Hx of previous constipation)<br></strong>no diet change, lives with anotehr dog and 2 young children<br><br>EDDU:<br>Heart worm prevention: Intestinal worming: Flea control: Tick control: has had SR12 from anotehr vet in Sept 25 and no current FT or worm tx<br>Diet: commerical<br>Current medications: no<br><br><strong>EXAMINATION</strong>:<br>MM: Pink and dry (8% dehydration)<br>CRT < 2 second<br>Demeanour: very quiet (usually crazy)<br>Hydration: dehydrated <br>Cardiovascular: Dual heart sounds auscultated, no murmur or arrhythmia detected, SSFP<br>Respiratory: Normal bronchovesicular sounds bilaterally, normal effort<br>Neurological: Appropriate & normal mentation, gait and posture<br>Musculoskeletal exam: will walk normally at home<br>Gastrointestinal: pain cranial abdomen , no cponstipation on abdominal palpation <br>Genito /Urinary: vuvla wnl<br>Integument: Coat condition good, no ectoparasites visualised <br>Eyes: Symmetrical, no ocular discharge or blepharospasm<br>Ears: Normal, no auricular discharge bilaterally<br>Mouth: Dental grade 1/4<br>Lymph Nodes: Submandibular, prescapular and popliteal palpate normally<br>Rectal: Normal anal glands, no masses, normal faeces with bit of mucous, no constipation<br><br>xrays abn gas pattern stomach and upper SI<br>bloods dehydration and stress/inflam<br><br><strong>PROBLEM LIST</strong>:<br>.V no D 3 days<br>dehydation<br><br><strong>ASSESSMENT</strong>:<br>.<br><br><strong>PLAN</strong>:<br>off to ARH for assessment for poss GIT FB<br>Revisit: Needed/not needed<br><br>.</p> </div> </div></html>
