01/03/2026 DATE/TIME of CONSULTATION: 21:29hrs, Sun 01/03/26 CLINICIAN: Dr Marlen Waterworth PRESENTING COMPLAINT: Milk chocolate ingestion HISTORY: 1 block of milk chocolate ingested 30-45 minutes ago had some wet food beforehand CLINICAL FINDINGS: Mentation:STRESSED! HR: 160 no m/a RR: MM: bright pink CRT:<1sec Temp 39.6 PROBLEM LIST: chocolate ingestion tachycardia pyrexia bright pink gums TREATMENT: half apomorphine right eye-vomited up copious amounts of melted light brown chocolate with paler looking wet food -flushed eye; gave metoclopramide 0.5 mg/kg SC CLIENT COMMUNICATION/PLAN: Gave handout on chocolate toxicosis and what to look out for. Might still have some vomiting/diarrhoea but it appeared everything was evacuated via emesis. O declined charcoal
