Reason:  5th amphotericin BSubjective:Improved over weekend after methadone + antinausea meds. Is much brighter today, still a little stiff with walking but otherwise EDDU normal. Did have mild D+ early hours of this morning. Also had facial swelling around the eyes at 2am this morning. Went to SASH, no obvious cause identified --> suspect contact allergies. Awaiting hx Current meds = - ampB 0.8mg/kg SC last given Fri- fluconazole 150mg po BID- theophylline 300mg PO SID (given last night)Objective:HR/PR:  88bpm, normal pulses, sinus arrhythmiaTemp: 38.3MM: Pink, 2s crtPulse:  normalResp: 20bpm, normal sounds and pattern. Not congested or stertorous on cxxHydration: nad Bloods collected: NeutrophiliaMild hypoglycaemiaAssessment: Fit for AmpB rxTreatment: AmpB 0.8mg/kg -> 12.5mg or 2.46mlInjected into NaCl 0.5%+ 2.5% glucose,  warmed to 40 degrees (achieved this by heating the 500ml bag in microwave for 60s).  Given via extension line injected with 22G needle under skin R and L flank fold.Infused entire bag in <20mins.Site massaged post-infusion.
