Reason: DFW'sSubjective: No meds, no other history. Was bitten by daschund and has injury to R side. Objective: BAR nervous but nice, mm pink, CRT 1 sec, HR 200+ initially settled to 168, pulses strong. R flank behind elbow is 12x15mm skin deep wound with small skin flap caudoventrally looking a little devitalised. Methadone IM, local spray, clipped, cleaned. Client Communication: discussed with O offered mild (conscious) sedation (gaba/traz) and attempted to remove skin flap + staple, will hopefully be successful, might need IM sedation if not. Vs leave to heal by 2nd intention, will take a few weeks.O decided to leave to heal by 2nd intentionPlan: keep clean, no baths / swimming / beach, no coverings or cream.
